Description
Provides financial assistance for various accessibility aids such as: prostheses, medical equipment, minor home modifications, mobility equipment, ramps, and wheelchairs. Works only with referring professionals (e.g. case managers, case workers, physicians, social workers) who apply on behalf of their clients and patients; individuals cannot apply directly.
