Application process

Complaint forms can be completed online and submitted on the Consumer Affairs website. Form can be downloaded, completed, and mailed to the address below. If downloading a form is not an option, a form can be mailed by calling 609-826-7100 to request a copy. Go to http://www.njconsumeraffairs.gov2. Select the "Boards and Committees" tab at the top of screen3. In the middle column, select "Medical Examiners"4. On the far left side, near the bottom, select "File BME Complaint"5. Complete all the required fields and press "Submit" at the bottom on the form
