Description
Provides home assessment of client to determine physical and psychosocial needs. A financial review is conducted to determine client's ability to pay for services and eligibility for supported services. Care Plan developed in response to client's needs. Fee depends on service desired. Services include benefits/budget counseling, care plan implementation and coordination and monthly case management.
