Description
Helps individuals remain in the community or return to the community rather than be cared for in a long term care facility. This medical assistance program offers limited covered services such as Home Health Services, Homemaker Services, Medical Day Care, Adult Social Day Care, Transportation Services, and Case Management. A federal home and community based service program.
