Application process

Call to complete an initial inquiry with an Intake Triage Specialist. Once the initial inquiry is complete, applicants can choose to come on their most convenient day during regular business hours at 9:00am. Applicants are asked to be prompt and to register with the receptionist at the front desk. Please call to schedule an appointment.

Fee

The program accepts most insurance plans, as well as self-pay with a sliding scale fee system available to residents of Somerset County.
