Guides individuals with diabetes through courses that incorporate their needs, goals, and life experiences, following evidence-based standards. Covers aspects such as the diabetes disease process, treatment options, nutrition management, physical activity, medication utilization, blood glucose monitoring, control improvement, acute and chronic complication prevention and treatment, goal setting, problem solving, psychosocial adjustment, and preconception care and pregnancy management if applicable.

Please call to schedule an appointment. A physician's referral will be required before attending the initial appointment.

Fee

Medicare and most private medical insurances are accepted. Amount of coverage varies by insurance plan. Medicare can cover 10 hours of the class, only once in a lifetime. Eligibility

Individuals with diabetes and their families, with particular focus on those who are newly diagnosed.
