Projects for Assistance in Transition from Homelessness (PATH) | Mental Health Association of Monmouth County
Remediates homelessness or imminent risk for adults with a mental health condition by providinglinkages to develop client stability. Services include: outreach, screening and diagnostic treatment, referrals for primary health care, employment and educational services, housing assistance, mental health and substance use disorder treatment, relationship building, access to financial assistance services such as general and emergency assistance, Medicaid, as well as transportation assistance. Individuals seeking services may have a co-occurring medical condition that has remained untreated; however, the program cannot accept new consumers who are already receiving other case management services.
