Description
Initiating as an extension of psychiatric rehabilitation programming, the Bridge to Wellness Integrated Care program facilitates access to primary care health services for individuals with psychiatric disorders, encompassing physical exams, diagnosis, and treatment of medical conditions (e.g., diabetes, hypertension, obesity), vaccinations, on-site laboratory services, wellness programs, health education, pap smears, and specialist referrals. Its aim is to eliminate barriers to critical primary care, mitigate hospitalizations, and preserve lives.
