Description
Integrated Case Management Services provides case management services designed to engage and support adults with serious and persistent mental illness, while striving to help them integrate into the community. Case Managers work with individuals to facilitate access to and utilization of needed resources and supports. ICMS Services include outreach, assessment, linkage to community resources, crisis intervention, substance abuse education and intervention, psycho-education, and advocacy.
