Assist families by providing a family-centric, community-based hub offering support, information, and services. Enhance the well-being of children and adults, fostering stronger families and neighborhoods through services like access to child, maternal, and family health information, linkage to publicly-funded health insurance programs, referrals to local healthcare services, development of plans for family stability, economic self-sufficiency, employment-related and income security services, job readiness training, information and referrals, life skills training (including budgeting and nutrition), housing assistance, parent education, parent-child activities, advocacy, and home visiting in accordance with the model of engagement approved by the Division of Prevention and Community Partnerships.
