Integrated Case Management Services | CPC Integrated Health
Assists adults with serious and persistent mental illness and/or co-occurring substance use disorders increase independence and promote community integration. Services provide linkages to help secure necessary services and resources while maintaining recovery and quality of life. Participants can receive services for up to 12 months.

Call to receive services. Individuals may also be referred by another treatment provider, including hospital discharge planners or outpatient providers.

Medicaid, Medicare and most private insurances. Grant funding available.
Eligibility
Adults with serious and persistent mental illness and/or co-occurring substance use disorders. Applicants must be a resident of either Monmouth or Ocean County.

With four counseling centers and a special education school, the agency promotes wellness, recovery, and productive lives.Providing services such as outpatient and in-home counseling, addiction recovery for teens and adults, medication-assisted treatment, ambulatory withdrawal management, integrated physical and mental health care coordination, anger management, mental health support groups, Program for Assertive Community Treatment (PACT), integrated case management, supported housing, supported employment, High Point Elementary and Adolescent Schools, children's mobile response, and school-based support services.
