Description
Provide individuals challenged by serious mental illness through the Program of Assertive Community Treatment (PACT). PACT aims to teach self-management, promote community integration, prevent hospitalizations, and enhance quality of life. Services, available 24/7, include personalized home-based support, medication management, crisis response, career counseling, and life skills training. PACT envisions positive outcomes like employment, education, supportive networks, improved family relations, and a substance-free life. Services are flexible, delivered in homes or community settings, with a lifelong option if needed.
