Application process

A provider referral and application form are required. Please visit the website to download a services application. Applicants may submit a completed application by mail, fax, or email. A provider referral can be obtained from the following places: - A Community Mental Health provider - A New Jersey State Psychiatric Hospitals - The Middlesex County Continuum of Care Coordinated AssessmentPlease call the office for more information and/or assistance.

Fee

The cost of supportive services is covered by Medicaid. The cost of housing is calculated as a fixed percentage of an individual's gross income.

Eligibility

To be eligible for supportive services with housing, applicants must meet the following criteria: - Have a diagnosed psychiatric illness (A physician's verification of disability is required) - Be in need of assistance to obtain affordable, safe housing - Be in need of supportive services to prevent hospitalization or institutionalization and maintain independence in the community
