[INTERNAL ONLY] Valley Home Care | Diabetes Self-Management Education Classes (DSME)
Offering Diabetes Self-Management Education Classes (DSME), this collaborative program equips individuals with diabetes to modify behaviors and autonomously manage the condition. Facilitated by licensed health professionals, the evidence-based course, conducted individually or in groups, addresses topics like disease processes, nutrition, and goal setting. The 4-week sessions last approximately 2 hours each.

A physician's referral will be required before registering. Those without a referral can receive assistance in obtaining one.Please call and ask to speak to Admission Services to begin registration.

Most medical insurances, including Medicare, are accepted. Amount of coverage varies by insurance plan. Please call for more information.Financial assistance may be available on a case by case basis. Please call and ask for the Billing Department for more information.
