Projects for Assistance in Transition from Homelessness (PATH) offers community-based case management and outreach services for adults who are dealing with a serious and persistent mental illness and are homeless or are at risk of becoming homeless. This program also provides targeted case management services for military veterans experiencing homelessness and mental health issues.

Eligibility
Individuals age 18 and older, suffering from serious mental illness, or suffering from serious mental illness and from substance abuse, and who are homeless or at imminent risk of becoming homeless.
