Eligibility

To the eligible for housing assistance, participants must be homeless as defined by the United States HUD guidelines AND have a household member with a disability as defined in Section 223 of the Social Security Act.

Centralized Intake by Social Services of Hunterdon, Sussex, and Warren. Must have referral from Tri-County COC.

Description

Offers permanent housing and case management services for individuals and families in which one adult or child has a disability, who are transitioning out of homelessness. The program offers clients supportive housing, enabling them to live as independently as possible.
